Housing (Admissions) Application

Applications for our recovery housing are submitted through our online application at housing.thelgfoundation.org. Depending on your answers, the application collects:

• Contact and identity information: first and last name, email address, phone number, date of birth, and gender
• Housing situation: where you are staying now, when you need housing, how long you are looking to stay, and what you are looking for in a recovery house
• Recovery and health information: whether you attend outpatient treatment and where, whether you take medication that supports your recovery (including medication-assisted treatment) and which medication, any arrangements you need to continue treatment, and any accommodations you need to participate
• Recovery support: whether you have a home group or sponsor, your sponsor’s phone number, and other support you have
• Employment, transportation, and how you plan to pay the program fee
• Legal supervision: whether you are on parole or probation, and your supervising officer’s name, phone number, and email address
• Any questions or other information you choose to share with us

Contact Form

When you use the contact form on our website, we collect:

• Your name, email address, and phone number (if provided)
• The content of your message

Donations

Online donations are made on a secure page hosted by PayPal. We do not see or store your card or bank account numbers. PayPal shares with us:

• Your name and email address
• The amount and date of your donation
• Any optional note you include, such as a dedication

Information Collected Automatically

Like most websites, our hosting and security providers automatically receive basic technical information when you visit, including:

• IP address, browser type, and device information
• The pages requested and the date and time of your visit

Housing Applications

Information from housing applications is used to:

• Determine whether our program is a good fit and whether a bed is available
• Prepare for your interview with our house manager
• Coordinate with your probation or parole officer when you are under supervision and have shared their information with us
• Maintain resident and program records required to operate a recovery residence

• Process your gift through PayPal and send you a receipt
• Thank you for your support
• Keep accurate financial records as required of a nonprofit organization

General Communications

We use contact form submissions and other messages to:

• Respond to your question or request
• Provide information about our programs and services

Website Operation and Security

• Deliver our website and keep it working correctly
• Detect and block spam, bots, and abuse of our forms

Service Providers

We use a small number of service providers to run our website, application, and communications. They process information on our behalf and only for these purposes:

• Cloudflare: hosts our website, processes contact form submissions, keeps a backup copy of submissions, and provides Turnstile bot protection on our forms.
• Netlify: hosts our online housing application and stores application drafts and submissions.
• Resend: delivers email notifications of contact form submissions and housing applications to our staff.
• Google Workspace: provides our staff email, where messages and application notifications are received.
• PayPal: processes online donations. PayPal’s use of your information is governed by its own privacy policy.

Probation, Parole, Courts, and Referral Sources

Many residents come to us through courts, probation and parole, and treatment programs. We share information with a referral source or supervising officer when you have authorized it, including through a signed release of information, or when we are required to by law or by the conditions of your supervision.

Legal Requirements and Safety

We may also disclose information when necessary to:

• Comply with a subpoena, court order, or other legal process
• Meet our obligations as a recovery residence under Pennsylvania law
• Protect the safety of our residents, staff, or the public in an emergency

We may share your information for other purposes when you give us permission.

No method of transmitting or storing information is completely secure, and we cannot guarantee absolute security.
